Métodos clínicos para la tracción de molares retenidos y su impacto en el éxito terapéutico y la duración del tratamiento: revisión narrativa

Resumen

Introducción: Los molares retenidos constituyen un trastorno eruptivo poco frecuente pero clínicamente relevante, asociado a complicaciones funcionales y oclusales si no se interviene oportunamente. Objetivos: Evaluar la evidencia reciente sobre los métodos clínicos empleados para la tracción de molares retenidos, con énfasis en sus tasas de éxito y duración del tratamiento. Materiales y Métodos: Se realizó una búsqueda electrónica en PubMed y ScienceDirect, incluyendo artículos originales en inglés publicados entre enero de 2019 y septiembre de 2025. Se seleccionaron once artículos relacionados con tracción de molares retenidos. Resultados: Los estudios mostraron tasas de éxito elevadas, que variaron entre 87,6% y 100%. Las técnicas asistidas con anclaje esquelético (particularmente el uso de minitornillos en rama mandibular e infracigomática) evidenciaron menor tiempo de tratamiento, con reducciones significativas frente a los métodos convencionales (11 semanas con minitornillos versus 24 semanas sin ellos). Los abordajes quirúrgicos y ortodóncicos combinados también mostraron resultados predecibles, aunque con tiempos más prolongados. Conclusiones: La evidencia disponible indica que los métodos de tracción asistidos con anclaje esquelético, especialmente el uso de minitornillos en la rama mandibular, ofrecen los mejores resultados en términos de éxito terapéutico y eficiencia, constituyendo la alternativa más favorable para el manejo de molares retenidos.
