Considerations for the diagnosis and management of calcified root canals. A narrative review

Root canal calcification is characterized by the deposit of mineralized tissue along the canal walls, it is usually asymptomatic and is detected incidentally during a radiographic examination. Teeth that suffer from this process may develop apical periodontitis over time and consequently require endodontic treatment. The pulp chamber and root canal space may be partially or completely obliterated, increasing the risk of procedural errors during root canal preparation. With the advent of cone beam computed tomography (HCCT), teeth can be visualized three-dimensionally, and the superimposition of anatomical structures is eliminated. This review aims to present the considerations for the diagnosis and management of calcified root canals. An electronic search was carried out in PubMed, SciELO, Google Scholar and Science Direct, using the keywords cone-beam computed tomography/endodontics /pulp canal calcification/root canal/guided endodontics, finally 40 articles were selected. It is concluded that the use of TCHC allows the precise assessment of the root canal system, as well as characterizing the calcified root canal, being considered valuable information in the planning of endodontic treatment, minimizing the risk of technical errors. The possibility of using endodontic guides has made it possible to reduce clinical times and complications, with more predictable therapeutic results.
