Posterior mandibular reconstruction using iliac graft and osteosynthesis material. A case report

Cemento-ossifying fibroma (COF) is a benign mesenchymal neoplasm characterized by slow, asymptomatic growth, originating from the periodontal ligament. It may cause bone deformities in advanced stages. The etiology remains incompletely defined, though it has been associated with trauma, periodontal infections, and surgical interventions. It predominantly affects young women aged 20–40, with a predilection for mandibular premolars and molars. Diagnosis is based on clinical, radiographic, and histopathological findings. Radiographically, it presents as well-defined radiopaque areas. Treatment varies based on lesion size and borders, ranging from surgical enucleation to segmental resection with bone reconstruction. A clinical case of a left posterior mandibular COF is presented, managed with complete resection and autologous bone grafting. Postoperative management included hyperbaric oxygen therapy, analgesics, and antibiotics.
