Rehabilitation of the completely edentulous mandibular patient represents a clinical challenge, especially in elderly adults, due to the limited retention and stability of conventional dentures. Mandibular overdentures retained by two implants in the interforaminal region constitute an effective and widely used therapeutic alternative in oral rehabilitation. We present the case of an 87-year-old male patient with controlled arterial hypertension, who presented with deterioration and maladaptation of his dental prostheses. Following clinical and radiographic evaluation, extraction of the lower teeth was indicated due to their unfavorable prognosis, followed by the placement of two implants in positions 34 and 43, and subsequent rehabilitation using a mandibular overdenture with a resilient axial retention system. Treatment included a surgical phase, an osseointegration period, delayed loading, and fabrication of the definitive prosthesis. During the initial clinical follow-up, adequate prosthetic stability and retention, favorable functional adaptation, and peri-implant tissues without clinical signs of biological complications were observed. In the present case, the two-implant mandibular overdenture allowed for a favorable functional rehabilitation during the initial follow-up in an elderly patient with a controlled systemic condition.
