Frenectomía e injerto gingival libre en recesión gingival profunda de incisivo inferior con fenotipo delgado: reporte de caso con seguimiento a 9 meses

La recesión gingival es una condición frecuente que puede comprometer la estética, generar hipersensibilidad dentinaria y favorecer la acumulación progresiva de biofilm dental. La frenectomía asociada a injerto gingival libre constituye una técnica predecible para incrementar el tejido queratinizado y lograr cobertura radicular parcial o total. El objetivo de este informe fue describir el manejo clínico para el aumento del ancho de encía queratinizada previo al tratamiento ortodóntico, con el fin de proporcionar estabilidad del tejido gingival. Se atendió a una paciente femenina de 26 años con diagnóstico de gingivitis inducida por placa, frenillo labial inferior con inserción mucogingival alta, asociado a recesión gingival tipo RT1 según Cairo en la pieza 3.1. El plan de tratamiento incluyó motivación e instrucción de higiene oral, junto con instrumentación supragingival. A los 7 días, se realizó la frenectomía, seguida de un injerto gingival libre para incrementar la encía queratinizada y estabilizar el margen gingival en la pieza 3.1. Al mes se observó contracción parcial del injerto. A los 9 meses, se evidenció una cobertura radicular del 80% y un aumento del ancho de encía queratinizada, con adecuada integración del tejido y ausencia de complicaciones posoperatorias. La combinación de frenectomía e injerto gingival libre constituye una alternativa terapéutica efectiva para el manejo de recesiones gingivales tipo RT1 en el sector anteroinferior, contribuyendo al incremento de la encía queratinizada y a la estabilidad del margen gingival previo al tratamiento ortodóntico.
